• Service fee is calculated and paid for at the time of ordering lab tests either online or with the phlebotomist.

What you need:

• lab order or TP Request form completed, signed and sent back from the ordering healthcare professional
• service fee is collected prior to procedure

What you need:

•requisition signed by physician along with Kit.

 •no service fee collected from patients having blood drawn at our lab that made an appointment through the MOMS platform online

• no lab orders needed
• Finger Stick 

• results in seconds

We draw for all specialty kits. If we are not contracted with them the service fee applies.

•The cost of the service fee includes: blood collection, proper handling and packaging, along with looking over all paperwork and ensuring the kits are left with the appropriate recipient to then be shipped back to the lab for testing. (FedEx, UPS, etc)
As a courtesy, patients will receive tracking information shortly after, so they may track their kit at any time.

•Pick-up contact (patient or healthcare professional) will need to sign upon pick up

•Drawn To You will pick up and transport specimen to designated lab whether the specimen collection was administered by a Drawn To You phlebotomist or not
